Important source distinctions

The handbook includes historical dosing below GFR 15 and during dialysis. The selected U.S. adult tablet label does not recommend use below eGFR 15 or during hemodialysis; the handbook is not an override.

Historical renal bands

GFR 30-60 mL/min: usual dose, or 25 mg with CYP3A inhibitors. At 15-29, the book lists 25 mg daily and avoidance with CYP3A inhibitors. It also lists 25 mg below 15, with the same interaction caution.

Historical renal-replacement comments

The book considers removal unlikely across APD/CAPD, HD, high-flux HDF and CAV/VVHD, referring the first three to <15 and CAV/VVHD to 15-29.

Source-specific limits

The selected U.S. adult tablet label does not recommend treatment below eGFR 15 or on hemodialysis. The book's below-15 and dialysis doses are historical source differences, not endorsed replacement protocols.

The original individual GlobalRPh pages commonly cite NIH/NLM DailyMed and instruct readers to review the applicable package insert for updates. That historical reference is preserved as context; it is not counted as an independently retrieved current label.
