Usual Dosing (Adults)
The GlobalRPh source states: "Removed from us market - may 2006" and lists historical systemic dosing 400 mg PO/IV daily. It states availability as ophthalmic drops only (Zymar).
Renal Dosing
CrCl >40 mL/min: no changes; <40: 400 mg once, then 200 mg daily.
Hemodialysis
HD: 400 mg once, then 200 mg daily; after dialysis on dialysis days. PD: 400 mg once, then 200 mg daily.
