Usual Dosing (Adults)
For mild to moderate Alzheimer-type dementia. Trials found 16 to 32 mg/day in two doses effective, but 32 mg was less tolerated without extra efficacy; recommended 16 to 24 mg/day divided twice daily. Although 24 mg was not statistically more beneficial than 16 mg, some individuals may benefit.
Start 4 mg twice daily for at least 4 weeks, then 8 mg twice daily. After at least 4 more weeks at 8 mg twice daily, may try 12 mg twice daily based on benefit/tolerability. Prefer morning/evening meals and ensure adequate fluid intake. After an interruption of several days or longer, restart at lowest dose and retitrate. Abrupt withdrawal did not increase adverse events versus continuation in the described studies, but benefits are lost after discontinuation.
Renal Dosing
Moderate renal impairment: generally do not exceed 16 mg/day. Severe impairment (CrCl <9 mL/min): not recommended.
Hemodialysis
Not recommended.
