Do not administer OPTIMARK in acute kidney injury or chronic severe kidney disease with GFR below 30. The source identifies NSF and renal adverse-effect concerns and advises screening before use. Renal dose adjustment has not been studied. Hemodialysis removal does not supersede the contraindication or establish protection from NSF.
