Renal dosing, restrictions and evidence limits

The renal subsection recommends no dose adjustment, while the warning separately identifies increased gadolinium exposure and nephrogenic systemic fibrosis risk in impaired elimination. Avoid use in patients at highest risk unless essential diagnostic information cannot be obtained by noncontrast MRI or another modality. Assess kidney function and avoid unnecessary repeat exposure. The agent is removable by hemodialysis, but removability does not make treatment risk-free. A no-adjustment statement is not equivalent to unrestricted eligibility for contrast administration.
