Restless legs syndrome

Take with food at about 5 PM. Cockcroft-Gault CrCl (mL/min): >=60, 600 mg daily; 30-59, start 300 mg daily and increase to 600 mg if needed; 15-29, 300 mg daily; <15, 300 mg every other day. RLS treatment is not recommended on HD.

Postherpetic neuralgia

At CrCl >=60: 600 mg each morning for 3 days, then twice daily. At 30-59: 300 mg each morning for 3 days, then twice daily; may increase to 600 mg twice daily. At 15-29: 300 mg on days 1 and 3, then every morning; may increase to twice daily. Below 15 without HD: 300 mg every other morning, increasing to daily if needed. On HD: 300 mg after each session, increasing to 600 mg if needed.

Withdrawal and formulation

When stopping PHN twice-daily treatment, reduce to once daily for one week. Swallow whole. HORIZANT is not substitutable with other gabapentin products.

Usual Dosing (Adults)

Indicated for moderate to severe primary restless legs syndrome (RLS) and postherpetic neuralgia (PHN) in adults. Swallow whole without cutting, crushing or chewing; take with food. Extended-release tablet strengths 300 and 600 mg.

RLS: 600 mg once daily at about 5 PM. 1200 mg/day provided no added benefit and more adverse reactions. If missed, take the next dose the following day as prescribed.

PHN: 600 mg each morning for 3 days, then 600 mg twice daily from day 4. More than 1200 mg/day provided no added benefit. Skip missed doses and take the next at its scheduled time.

Adult restless legs syndrome: CrCl >=60 mL/min, 600 mg daily; 30-59, start 300 mg daily and increase to 600 mg if needed; 15-29, 300 mg daily; <15, 300 mg every other day. For hemodialysis patients, use for RLS is not recommended.

Adult postherpetic neuralgia: the renal table instead permits 300 mg after each dialysis, increasing to 600 mg if needed. Non-dialysis PHN maintenance is 600 mg twice daily at >=60; 300 mg twice daily at 30-59 (may increase to 600 mg twice daily); 300 mg each morning at 15-29 (may increase to twice daily); and 300 mg every other morning at <15 (may increase to daily). PHN has separate titration and discontinuation instructions: consult the complete table before starting or stopping.

Take this extended-release product with food and swallow whole. These instructions are not interchangeable with other gabapentin products.
