Adult C. difficile-associated diarrhea: 200 mg orally twice daily for 10 days, with or without food. No dose adjustment is recommended based on renal function, including the severe-impairment category studied (CrCl <=30 mL/min). The label does not provide a distinct dialysis replacement dose.

This is an oral intestinal-infection regimen, not treatment of systemic infection. Do not substitute the pediatric suspension volume table without confirming age, weight and concentration.

Historical renal / replacement-therapy guidance

The 20-50, 10-20 and <10 mL/min bands retain the usual indication-specific dose. The <10 band adds caution. The adult C. difficile regimen is 200 mg twice daily for 10 days. PD, HD, high-flux and continuous therapy retain usual dosing; removal is considered unlikely. The book notes older UK caution below 30 because of limited data, while its renal dose recommendation draws on U.S. information.

Dosing / renal protocol and limits

For adult C. difficile infection, the dose is 200 mg orally every 12 hours. UCSF retains this schedule in renal dysfunction and hemodialysis. Select the CDI treatment course under the applicable infection guideline; this is not systemic antibacterial treatment.
