Renal dosing, restrictions and evidence limits

Adults start 4 mg once daily. The renal table allows 8 mg daily at CrCl 30-89 and 4 mg daily at 15-29 or <15; escalation still depends on response and tolerability.

Children >25 through 35 kg: eGFR 30-89, 4 mg daily; below 30 or dialysis, not recommended. Children >35 kg: eGFR 30-89, start 4 mg daily and increase to 8 mg after one week; 15-29, 4 mg daily; below 15 or dialysis, not recommended.

Strong CYP3A4 inhibitors add separate restrictions. Pediatric renal doses were derived using assumed proportional renal effects. Do not apply the adult <15 row to children.

Usual Dosing (Adults)

Muscarinic antagonist for overactive bladder with urge incontinence, urgency and frequency. Start 4 mg daily; may increase to 8 mg daily based on response/tolerability. Maximum 4 mg/day in severe renal impairment (CrCl <30 mL/min) or with potent CYP3A4 inhibitors such as ketoconazole, itraconazole or clarithromycin. Not recommended in severe hepatic impairment (Child-Pugh C). Swallow whole with liquid, with or without food; do not chew, divide or crush.

4 mg extended-release tablets: light blue, oval, biconvex, film-coated, engraved FS. 8 mg extended-release: blue, oval, biconvex, film-coated, engraved FT.
