Renal dosing, restrictions and evidence limits
Avoid use in severe renal impairment. For mild-to-moderate impairment, begin with one-half of the usual starting patch dosage and titrate carefully, monitoring sedation and respiratory depression at each increase. This does not permit use in an opioid-naive patient, for acute pain, or as an as-needed analgesic. Complete opioid conversion and product eligibility requirements remain necessary.
