eGFR 30 to <60 mL/min/1.73 m2: start 35 mg orally once daily. Increase only after assessing renal function and triglyceride response. Use is contraindicated at eGFR <30 mL/min/1.73 m2, including ESRD and dialysis. Assess renal function before treatment and periodically during therapy.
