Renal dosing, restrictions and evidence limits

At CrCl 15-29 mL/min, reduce to 200 mg orally once daily. No renal starting-dose change is recommended at CrCl 30-89, but moderate impairment requires more intensive adverse-effect monitoring and later toxicity-based changes when needed. Baseline and ongoing thiamine, blood counts, kidney/liver tests and pancreatic enzymes remain relevant. No regimen is extrapolated below 15 mL/min or to dialysis from these bands.

Renal dosing, restrictions and evidence limits

CrCl 15-29 mL/min: reduce to 200 mg orally once daily. No starting-dose change is recommended for CrCl 30-89 mL/min; patients in the moderate range require closer safety monitoring and dose modification for adverse reactions. No ESRD regimen is established by these selected sections. A renal reduction and an interaction-related reduction are not automatically cumulative.
