Renal dosing, restrictions and evidence limits

CLcr 30-89 mL/min does not require renal dose modification. For CLcr 15-29 mL/min, limit ULORIC to 40 mg once daily. Dialysis patients were not studied. The usual adult starting dose is 40 mg daily; the severe-impairment limit prevents routine escalation to 80 mg. The source restricts its gout indication to patients with inadequate response to maximally titrated allopurinol, intolerance, or a reason allopurinol is not advisable, and warns about increased cardiovascular-death risk. It is not recommended for asymptomatic hyperuricemia.
