Usual Dosing (Adults)
20 mg orally/IV every 12 hours or 40 mg at bedtime.
Renal Dosing
CrCl >50 mL/min: no change. CrCl 0 to 49: decrease dose by 50% (e.g. 10 mg every 12 hours or 20 mg at bedtime), or extend interval to 36 to 48 hours to limit CNS adverse effects.
In adult moderate (CrCl <50) or severe (<10) impairment, elimination half-life increases; in severe impairment it may exceed 20 hours, approximately 24 hours in anuria. CNS adverse effects have been reported. To avoid accumulation, reduce famotidine injection dose by half or extend interval to 36 to 48 hours according to clinical response. Comparison of adult/pediatric pharmacokinetics supports considering adjustment in pediatric moderate/severe impairment.
Hemodialysis
Reduce dose 50% (e.g. 10 mg every 12 hours or 20 mg at bedtime) or extend to every 36 to 48 hours. Give after dialysis on dialysis days.
