Renal dosing, restrictions and evidence limits

Use 75 mg twice daily for moderate or severe renal impairment, defined by eGFR below 60, and for ESRD below 15 in patients not yet receiving dialysis. Mild impairment does not require adjustment. The nondialysis ESRD finding is not a validated dialysis recommendation. Full gallbladder, pancreatic, hepatic and interaction restrictions must still be checked before using this regimen.

For eGFR below 60 mL/min/1.73 m2, including end-stage renal disease not yet on dialysis, the recommended dose is 75 mg twice daily with food. No adjustment is needed for mild impairment. The recommendation explicitly describes ESRD not yet on dialysis and must not be converted into a dialysis regimen.
