Renal dosing, restrictions and evidence limits
Immune-mediated nephritis is a toxicity-management problem, not merely a renal-clearance dose calculation. The label withholds treatment for Grade 2 or 3 creatinine increase and permanently discontinues for Grade 4. Restart eligibility, corticosteroid treatment and recurrent toxicity require the complete oncology algorithm. The selected pharmacokinetic paragraph reports no clinically significant difference at eGFR at least 15 mL/min/1.73 m2 but inconsistently calls the measure estimated creatinine clearance. Preserve that source ambiguity rather than converting units. These pharmacokinetic findings do not supply a dialysis regimen or eliminate the nephritis warning.
