The original page lists controlled-release usual doses but does not explicitly identify a controlled-release renal algorithm. Do not transfer immediate-release interval recommendations to controlled-release products.
Usual Dosing (Adults)
Weight <50 kg: 100 mg every 6 hours or controlled-release 200 mg every 12 hours. Weight >50 kg: 150 mg every 6 hours or controlled-release 300 mg every 12 hours. With no response, may increase to 200 mg every 6 hours. The source maximum required for severe refractory ventricular tachycardia is 400 mg every 6 hours.
Renal Dosing
CrCl >40 mL/min: no change; 30 to 40: 100 mg every 8 hours; 15 to 30: 100 mg every 12 hours; <15: 100 mg every 24 hours.
Alternative source strategy: CrCl 30 to 40, reduce dose 50%; CrCl 15 to 30, reduce dose 75%.
Hemodialysis
100 mg every 24 hours. Not dialyzable.
