KORSUVA uses target dry body weight and is administered after blood stops passing through the dialyzer. A missed session resumes at the next HD session.

Before applying: Do not use predialysis fluid-loaded weight by default, give before completion of dialysis, or extrapolate to PD/non-HD CKD.

Renal dosing, restrictions and evidence limits

Give 0.5 mcg/kg IV into the venous dialysis line at the end of each HD session, using target dry body weight. Administer after blood has stopped circulating through the dialyzer; the drug is dialyzable. If HD is missed, resume at the end of the next treatment rather than adding a replacement dose outside that schedule. Do not transfer this regimen to nondialysis CKD or peritoneal dialysis.
