This fixed combination includes saxagliptin and metformin. Its selected label version has a product-specific renal restriction; a lower threshold for a different dapagliflozin indication does not override it.

This May 2019 source permits no renal dosage modification at eGFR at least 45 mL/min/1.73 m2 and contraindicates this fixed combination below 45. Assess renal function before use and periodically, and correct volume depletion. The contrast section directs interruption for the specified liver disease, alcoholism, heart-failure or intra-arterial-contrast circumstances, with renal reassessment after 48 hours before restarting if stable. The three-component restriction must not be replaced by a single component's renal indication or threshold. Current labeling and availability require reconciliation before prescribing from this older source.
