Renal dosing, restrictions and evidence limits
For adults with NSCLC or IMT, no renal dose change is recommended at CrCl 30-89 mL/min. With severe impairment (CrCl <30) not requiring dialysis, use the second dose-reduction level: 250 mg orally once daily. The renal section refers explicitly to the dose-reduction table. Dialysis is outside this recommendation. Pediatric ALCL/IMT and young-adult ALCL use different body-surface-area tables; the adult fixed dose must not be transferred to those populations. The source uses modified Cockcroft-Gault for adults and Schwartz for pediatric assessment.
