Usual Dosing (Adults)

Colistimethate sodium injection: vials equivalent to 150 mg colistin base activity (CBA). Reconstitute each vial with 2 mL sterile water for injection, giving 75 mg/mL CBA. Swirl gently to avoid frothing; inspect for particles or discoloration and do not use affected product.

Adults and pediatric patients, IV or IM: 2.5 to 5 mg/kg/day in 2 to 4 divided doses with normal renal function, depending on infection severity. In obesity base dosage on ideal body weight. Reduce daily dosage in renal impairment.

Renal Dosing
