Renal dosing, restrictions and evidence limits
Cobicistat can lower creatinine-based estimates by inhibiting tubular creatinine secretion without reducing measured glomerular function. The mirror nevertheless calls for close renal-safety monitoring when a confirmed serum-creatinine increase exceeds 0.4 mg/dL from baseline.
TYBOST with tenofovir disoproxil fumarate is not recommended below estimated CrCl 70 mL/min in this source. Renal injury, including Fanconi syndrome, has been reported with TDF-containing regimens. With TDF, assess urine glucose/protein and estimated clearance at baseline and during treatment; assess phosphorus when renal impairment or risk is present.
Do not presume every creatinine rise is benign or transfer this TDF-specific rule to TAF combinations.
