Renal dosing, restrictions and evidence limits

CrCl below 30 mL/min is a contraindication because of magnesium accumulation. In mild/moderate impairment or with diuretics, ACE inhibitors, ARBs or NSAIDs, renal-injury risk warrants caution; consider baseline and post-colonoscopy electrolytes, creatinine and BUN.

Correct fluid/electrolyte abnormalities beforehand. Maintain appropriate hydration before, during and after preparation, but use caution with fluid replacement in congestive heart failure. Clear liquids should include balanced electrolyte solutions rather than water alone. Significant vomiting or dehydration also warrants consideration of post-procedure laboratory testing.

The mirror contraindicates use at CrCl below 30 mL/min because magnesium may accumulate. Lesser renal impairment and concomitant medicines affecting kidney function require caution, hydration assessment and consideration of laboratory monitoring.
