Renal dosing, restrictions and evidence limits
Renal insufficiency can alter drug disposition and reduce gluconeogenesis, increasing the risk of severe or prolonged hypoglycemia. The mirror directs conservative initial and maintenance dosing in renal impairment but supplies no numeric renal bands.
Its lower starting-dose example is specifically geriatric, not a validated CrCl-based renal schedule; it is not recast here as one. Long drug persistence can require prolonged supervision after hypoglycemia. Current product labeling and the appropriateness of this older therapy remain unverified in this pass.
