Usual adult dosing

Usual infections: 750 mg to 1.5 g every 8 hours. Severe/complicated or bone/joint infections: 1.5 g every 8 hours. Life-threatening infection may require 1.5 g every 6 hours; meningitis dosing must not exceed 3 g every 8 hours.

Renal adjustment

CrCl >20 mL/min: 750 mg to 1.5 g every 8 hours; 10-20: 750 mg every 12 hours; <10: 750 mg every 24 hours. The label directs an additional dose at the end of HD because cefuroxime is dialyzable. Do not apply the oral axetil schedule to this injectable product.

Renal dosing, restrictions and evidence limits

Adult injection doses: CrCl above 20 mL/min, 750 mg to 1.5 g every eight hours; CrCl 10-20, 750 mg every twelve hours; below 10, 750 mg every twenty-four hours. The source calls for a further dose after each hemodialysis session but does not specify that supplemental amount in the selected footnote. Its creatinine-clearance equation assumes steady-state serum creatinine. These are injection instructions, not the oral cefuroxime-axetil schedule.
