Dosing / renal protocol and limits
Adult IV regimens are not routinely reduced for kidney dysfunction or hemodialysis in this source. Standard treatment uses 2 g daily; UTI uses 1 g daily. Meningitis and enterococcal endocarditis treated with concomitant ampicillin use 2 g every 12 hours. Obesity (BMI >30) can warrant individualized higher dosing with ID/pharmacy input. The indication-specific regimen is retained rather than treating all renal patients as receiving one dose.
