Usual Dosing (Adults)

1 to 2 grams every 12 hours; life-threatening infections: 3 grams IV every 12 hours. Maximum 6 g/day.

For K. pneumoniae skin infections, the source specifies 1 or 2 g every 12 hours IV/IM. If C. trachomatis is suspected in gynecologic infections, add anti-chlamydial coverage; cefotetan has no activity against it.

Prophylaxis for clean-contaminated or potentially contaminated surgery in adults: 1 or 2 g IV once 30 to 60 minutes before surgery; for cesarean section, give as soon as the cord is clamped.
