Renal dosing, restrictions and evidence limits

Assess renal function before treatment and periodically thereafter. The selected paragraph calls for greater toxicity surveillance in compromised renal function and discontinuation when CrCl is below 10 mL/min. It also contains a broader instruction not to administer in compromised renal function.
