Renal dosing, restrictions and evidence limits

The mirror associates CrCl below 60 mL/min with increased severe myelosuppression. Below 15 mL/min, data are insufficient for a treatment recommendation. Initial-course guidance must not replace later adjustment to marrow tolerance.

It describes the Calvert relationship: total dose in mg = target AUC x (GFR in mL/min + 25), not mg/m2. The underlying studies used measured Cr-EDTA clearance. This is not authorization to substitute any reported eGFR or select an AUC.
