Renal dosing, restrictions and evidence limits
Mild renal impairment needs no dose adjustment in this source, but requires close monitoring because the sulfobutylether beta-cyclodextrin vehicle can accumulate. CARNEXIV should generally not be used in moderate or severe renal impairment. The selected renal subsection does not supply numeric severity cutoffs.
Monitor renal function and electrolytes. Use beyond seven days has not been studied. Do not extend this IV vehicle restriction to all oral carbamazepine formulations, or invent a reduced IV regimen to bypass it. A retrieved historical monograph is not proof that the product is currently commercially available.
