Keep Cockcroft-Gault and the unstudied severe range explicit

The no-adjustment finding is limited to the selected Cockcroft-Gault mild/moderate renal range.

Before applying: Do not convert an unstudied severe range into a dose or silently substitute indexed eGFR.

Renal dosing, restrictions and evidence limits

No adjustment is recommended with mild or moderate impairment, Cockcroft-Gault CrCl 30-89 mL/min. Severe impairment at CrCl 15-29 mL/min has not been studied. This no-adjustment statement does not establish a regimen below 30 or during dialysis.

Keep the specified Cockcroft-Gault measure attached to the finding. Do not use the renal paragraph to select an oncologic starting dose or override toxicity- and interaction-related modification rules.
