Renal dosing, restrictions and evidence limits
The selected indication is adult CKD stage 3 or 4 with total 25-hydroxyvitamin D below 30 ng/mL. It is not indicated for CKD stage 5 or ESRD on dialysis. Before starting, serum calcium should be below 9.8 mg/dL.
Start 30 mcg orally at bedtime. After approximately three months, 60 mcg at bedtime may be used if PTH remains above the desired range and calcium is below 9.8 mg/dL, phosphorus below 5.5 mg/dL and total 25-hydroxyvitamin D below 100 ng/mL. Monitor these measures and PTH; suspend for persistently low PTH, hypercalcemia or persistently excessive 25-hydroxyvitamin D. This is a titration within a defined CKD indication, not a universal vitamin D renal dose.
