Renal dosing, restrictions and evidence limits
CRYSVITA is contraindicated with severe renal impairment or ESRD. The source defines severe impairment as pediatric eGFR 15-29 mL/min/1.73 m2 or adult CrCl 15-29 mL/min; ESRD is below 15 using the respective measure.
Renal impairment can magnify phosphate increases, causing hyperphosphatemia and nephrocalcinosis; renal PK effects are unknown. Concomitant oral phosphate or active vitamin D analogs and normal/high age-adjusted serum phosphorus are separate contraindications. Do not use the adult clearance unit in a child's indexed-eGFR restriction or turn unknown renal PK into a reduction schedule.
