1 additional source qualifications - review before use
Renal cautions are not interchangeable release-form schedules

SR and XL renal excerpts both recommend considering a lower dose and/or frequency, but do not provide a complete numeric renal schedule. Their release forms and ordinary administration schedules remain separate.

Before applying: Open the correct release-form record. Do not invent a shared renal interval or transfer a single-dose limit from another formulation.

Also check: Renal cautions are not interchangeable release-form schedules

Renal dosing, restrictions and evidence limits

For GFR below 90 mL/min, consider reducing the XL dose and/or administration frequency. Renally cleared metabolites can accumulate; monitor for exposure-related adverse effects. No universal renal tablet strength or interval is supplied. The adjacent hepatic dosing instructions are not a renal algorithm, and SR and XL should remain formulation-specific.

formulation separation

Renal cautions are not interchangeable release-form schedules

SR and XL renal excerpts both recommend considering a lower dose and/or frequency, but do not provide a complete numeric renal schedule. Their release forms and ordinary administration schedules remain separate.

How to use this: Open the correct release-form record. Do not invent a shared renal interval or transfer a single-dose limit from another formulation.

Renal dosing, restrictions and evidence limits

For renal impairment with GFR below 90 mL/min, consider reducing the dose and/or dosing frequency. The renal subsection does not supply a fixed percentage reduction or interval table. Do not misapply the separate hepatic-impairment schedule as the renal schedule.
