Renal dosing, restrictions and evidence limits

For CrCl 30 to 50 mL/min, the adult starting dose is 300 mg once daily for newly diagnosed chronic-phase CML, or 400 mg once daily for previously treated resistant/intolerant disease. For CrCl below 30 mL/min, the corresponding doses are 200 mg and 300 mg once daily. The reference starting doses without renal or hepatic impairment are 400 mg and 500 mg once daily, respectively. Hemodialysis has not been studied. Monitor renal function and use the full label for subsequent toxicity-related changes.
