Renal dosing, restrictions and evidence limits
At CrCl <30 mL/min, MALARONE is contraindicated for malaria prophylaxis. Treatment is a separate decision: use cautiously only when the benefits of the three-day regimen outweigh increased-exposure risks. Mild/moderate impairment needs no adjustment, but the source reports no long-term prophylaxis data beyond two months in moderate impairment. Do not transfer atovaquone-alone guidance to this proguanil-containing combination.
