ONAPGO: initial extra dose differs from continuous and later doses

The May 2026 official renal instruction specifically limits the initial extra dose. The older RX rendering supplies PK information without that usable prescribing subsection.

Before applying: Use the complete official product instructions; do not lower every dose automatically or substitute intermittent apomorphine.

Also check: ONAPGO: initial extra dose differs from continuous and later doses

Product / population
ONAPGO 98 mg/20 mL (4.9 mg/mL) solution for continuous subcutaneous infusion in adults with advanced Parkinson disease; not an intermittent apomorphine product. Selected renal sections, not a complete pump prescription.
Renal measure
Mild/moderate renal impairment as named in the source; no numerical renal-function cutoff is provided in the selected dosing subsection.

Renal dosing, restrictions and evidence limits

For mild or moderate renal impairment, the initial extra dose is 0.5-1 mg and must not exceed 1 mg. This renal restriction applies to the initial extra dose: no renal adjustment is needed for the continuous dose, maximum recommended daily dose, or subsequent extra doses. Severe impairment has not been studied. ONAPGO is not substitutable for intermittent apomorphine products. Apply the complete pump training, titration, total-dose, adverse-effect and interaction safeguards; this is not a stand-alone infusion order.

Renal dosing, restrictions and evidence limits

A single-subcutaneous-dose renal PK study found increased apomorphine exposure in moderate impairment; severe impairment was not studied. The retrieved February 2025 RX rendering lacks a usable renal prescribing subsection. This is selected PK content, not the initial-extra-dose rule. A separately stored May 2026 DailyMed label supplies the renal prescribing instructions; it does not repair the RX rendering.
