Labeled basic dosing

Give 1 mg orally once daily, with or without food. Duration is indication-specific and should follow the oncology treatment plan.

Renal information and dialysis

The selected U.S. label requires no dose change for renal impairment because renal dysfunction does not materially affect total-body clearance.

Selected warnings and limits

Review bone-density loss and cholesterol effects. Estrogen-containing therapy can oppose the treatment effect; concurrent tamoxifen is not recommended.

Historical renal and dialysis observations

The book retains standard dosing in all three renal bands and its listed dialysis modalities. Its commentary explicitly contrasts U.S. no-reduction advice with an older UK recommendation to avoid use below 20 mL/min.
