Renal dosing, restrictions and evidence limits
No adjustment is recommended at CrCl 30-89 mL/min. Severe impairment below 30 mL/min and ESRD have not been studied.
Renal impairment during therapy, including fatal events, is a separate warning. For Grade 3 renal toxicity, withhold until serum creatinine is 1.5 times the upper limit of normal or less, then resume at a reduced dose. Grade 4 renal toxicity requires permanent discontinuation. The initial no-adjustment band does not negate these treatment-emergent rules.
