Renal dosing, restrictions and evidence limits

Adult baseline renal impairment: 10 mg orally every 24 hours at CrCl >=50 mL/min, every 48 hours at 30-49, and every 72 hours at 10-29. Hemodialysis: 10 mg every seven days after dialysis. The table specifies Cockcroft-Gault with lean or ideal body weight.

No regimen is available below 10 mL/min without hemodialysis or for adolescents with renal insufficiency. These interval adjustments were not clinically validated for safety/effectiveness and were derived from pre-existing renal impairment; they may not apply when renal injury develops during treatment. Follow renal function and antiviral response closely. Peritoneal-dialysis removal was not evaluated.
