Renal dosing, restrictions and evidence limits

The source expects no clinically significant pharmacokinetic difference across mild-to-severe impairment, CrCl 15 to <90. Do not convert that expectation into a tested dialysis regimen or extend it below the stated range. Treatment-emergent kidney injury is a separate safety question.
