Renal dosing, restrictions and evidence limits

At serum creatinine above 1.8 mg/dL, the source limits the daily total to 1 g, administered every 12 hours. It does not define the optimal further reduction or interval as renal function worsens. The contraindication applies when serum creatinine exceeds 2.5 mg/dL and/or CrCl is below 20 mL/min.

The label advises CBC and reticulocyte assessment after two weeks and at three-month intervals; a reticulocyte count above 6% warrants consideration of dose reduction.
