Renal dosing, restrictions and evidence limits

The retrieved table specifies 100 mg once daily for mild impairment (MDRD eGFR 60-89, printed mL/min) and 50 mg once daily for moderate impairment (30-59); it allows doubling the initial dose for inadequate response in those groups. Severe impairment (15-29) and ESRD, including renal replacement therapy, are not recommended.

The selected source reports no renal-replacement study and notes that phase 3 atopic-dermatitis trials excluded baseline CrCl below 40 mL/min.
