Renal dosing, restrictions and evidence limits

For indexed eGFR 15-59 mL/min/1.73 m2 or end-stage disease receiving hemodialysis, start pitavastatin 1 mg once daily and do not exceed 2 mg daily. ZYPITAMAG is not available as a 1 mg tablet: use a different pitavastatin formulation for that starting dose, rather than inventing a tablet-splitting instruction. Mild renal impairment has no specific adjustment recommendation in this source.
