Renal dosing, restrictions and evidence limits
The retrieved RX label mirror uses eGFR, not the older local CrCl cutoff: below 45 mL/min/1.73 m2, use 2.5 mg orally once daily, including ESRD on hemodialysis. Give after hemodialysis; peritoneal dialysis was not studied. At eGFR >=45, the renal recommendation is the same as for normal function. Assess kidney function before starting and as clinically indicated. The preserved CrCl <=50 statement and this indexed-eGFR <45 statement must remain separately attributed, not combined into one threshold.
