Dosing / renal protocol and limits

Adult dose: 1 mg/kg IV every 12 hours using total body weight. No routine reduction is recommended for renal dysfunction or hemodialysis. With a strong CYP3A4 inducer, the listed regimen is 1.5 mg/kg every 12 hours.

Severe hepatic impairment is a separate adjustment: 1 mg/kg every 12 hours for two doses, then every 24 hours. Do not confuse hepatic or interaction adjustments with kidney-function dosing.
