Clinical context remains essential

Independent clinical review is pending. Confirm indication, exact formulation/route, population, renal measure and units, kidney-function stability, dialysis prescription, interacting drugs, monitoring and source revision. An RX mirror and its original label are not independent corroboration. Missing disagreement notes do not certify agreement.

IV vehicle risk is separate from oral renal dosing. Historical interactions and hepatic-study limits require exact current-label review; the preserved text is not an interaction-checking algorithm.

IV vehicle warning versus qualified institutional short-course use

VFEND favors oral therapy below its renal threshold unless an individualized benefit-risk assessment justifies IV use. UCSF acknowledges that warning and discusses limited evidence for short IV courses in selected patients. The guidance differs in emphasis, not through removal of the vehicle warning.

What to reconcile: Check the exact IV product and excipient, oral feasibility, intended duration, creatinine trend, interactions and drug-level monitoring. Neither source supports ignoring renal deterioration or automatically reducing the oral dose.

Source-specific interpretation; independent clinician adjudication pending.

Dosing / renal protocol and limits

Adult treatment: 6 mg/kg IV/orally every 12 hours for two doses, then 4 mg/kg every 12 hours. Heme/BMT prevention: 200 mg every 12 hours; obesity may warrant 4 mg/kg every 12 hours using adjusted weight and specialist review. The same regimens are listed for hemodialysis without a renal dose reduction.

For weight-based doses, use total weight unless >120% of ideal, when UCSF uses adjusted weight. Oral doses need practical tablet/suspension rounding and drug-level guidance.

The source separately notes the manufacturer's renal warning about accumulation of the IV sulfobutylether-beta-cyclodextrin vehicle. Limited studies informing short IV courses do not eliminate that product-specific risk. Oral and IV risk assessments must remain separate; review interactions and levels.
