NESINA: single-agent renal dosing is not combination eligibility

Alogliptin renal doses and HD timing do not determine eligibility for an alogliptin/metformin fixed combination. Peritoneal dialysis remains unstudied.

Renal dosing, restrictions and evidence limits

Assess renal function before treatment and periodically. At CrCl >=60 mL/min, no adjustment is needed. At CrCl >=30 to <60, use 12.5 mg once daily. At CrCl >=15 to <30 or ESRD (<15 or hemodialysis), use 6.25 mg once daily. Dosing need not be timed to hemodialysis. Peritoneal dialysis has not been studied. This single-agent guidance is not the renal eligibility rule for an alogliptin/metformin fixed combination.
