Dosing / renal protocol and limits

Oral cefuroxime axetil: CrCl >30 mL/min, 500 mg twice daily; 10-29, 250 mg twice daily; <10, 250 mg daily. Exactly 30 is not assigned by the displayed ranges.

Intermittent HD: 250 mg orally daily. Continuous-HD dosing is not supplied. This protocol cannot be used for injectable cefuroxime sodium; the separate injection page has its own guidance.
