Dosing / renal protocol and limits

Adult baseline 2.5 g IV every 8 hours; all doses over 2 hours. CrCl 31-50: 1.25 g every 8 hours; 16-30: 0.94 g every 12 hours; 6-15: 0.94 g daily; <=5: 0.94 g every 48 hours. The separate HD/PD row uses 0.94 g daily with limited data, after dialysis on HD days. This dialysis interval differs from the U.S. label and is not silently reconciled.
