Dosing / renal protocol and limits

For adult community-acquired pneumonia in intensive care, use 500 mg IV/orally daily. Outside intensive care, the listed regimen is 500 mg once followed by 250 mg daily. UCSF does not change these regimens for renal dysfunction or dialysis. These are pneumonia regimens, not doses for every azithromycin indication.
